Case Management Experience

The role of case management is to help in the planning of care provided to the patients and also to ensure that the patients receive the assistance they need. The case management experience occurred when one of my close relatives was taken to the hospital because of complications of diabetes. Upon admission of the patient at the hospital, a nurse case manager investigated the condition of the patient with the assistance of other medical personnel, who formulated a treatment plan. The case manager helped in the organisation of the patient and made sure that the healthcare team shared important information. It was also the case manager who helped in the process of the discharge. It included post-discharge appointments, educational efforts of teaching the patient about medication, dieting, and home care of their health as well (Giardino & Jesus, 2023). This care would be used in retaining the patient under care after leaving the hospital. Case management takes care of patients who are on the spectrum between hospital management and community/ home management.

Course Learning reflection.

This course helped me to understand that the process of case management is based on the principles of communication and working in a team, as well as a patient-centered approach. The medical workers are invited to collaborate and exchange their knowledge to be able to deliver quality care. In my situation, the negotiation of the case held between the healthcare department and the family of the patient was clearly described by the case manager (Katantha et al., 2025). Through this collaboration, every one of them was familiar with care planning and helped a patient. Coordinated care would optimize patient outcomes, and complications would be reduced.

Possible Improvements

Although the practice was effective, one of the possible ways to optimize the practice would be to increase the post-discharge follow-up. To refer to an example, the healthcare providers can make a call and discuss with the patient or accomplish a task of telehealth to monitor his/her progress. This would help in making sure that the patient goes through the treatment plan and takes up the drugs as he/she is supposed to. It also reduces the chances of the patient’s rehospitalization. In total, appropriate case management is applicable in terms of the improvement of patient outcomes and continuity of care.
