The Flashback of My Nurse Role in the world.
As a professional nurse in the contemporary healthcare environment, one has to be a global citizen. The health care systems are prone to all kinds of vagaries including economic crises, shortage of workforce, emergence of new diseases and heightening of health disparities among others. I also need to understand that clinical factors are not interrelated and dictate the care provided to the patient independent of other aspects such as ethics, laws, culture, and social factors. The issue is the condition of the patient. In this instance, cross-border, cross-cultural, and cross-differentiative processes play a critical role in defining the relevance of global citizenship in the nursing fraternity. It is also human, noble and just to every human being; in its cause; it tries to provide good health care to the entire people without being discriminative. ANA Code of Ethics is a code of ethics conduct, where good memory should be adhered with by ensuring that the nurses comply with the code of ethics to maintain the dignity of any patient rendered to them, to provide the autonomy of their own patients and to deliver justice in any kind of care. The ANA Code can also become an important part of the ethical nursing practice, as the investigations by Numminen et al. (2024) prove it, and the level of professional responsibility can be enhanced. The existing ethical standards are the ones I can understand and bring to practice since culturally sensitive and patient-centred care has become more practical.
My worldview was developed when I started to study BSN.
The BSN program has helped to make the world a bigger place in a number of ways. First, I have gained new information about such social determinants of health and the social determinants of health. As a matter of fact, I too was similarly careless in inquiring on the process and treatment plan of the disease in the past. Nevertheless, over the course of time, I realized that there are things that are far out of reach of a patient and equally, they are the ones that determine whether a patient will fall into the category of a compliant care plan or not. The variables that could have led to such monumental influence on the outcome of health include transportation, income, level of education and a stable house. This is where I have given the example of chronically ill patients who were not prepared to visit the clinic, because they do not want to. This, though, can hardly be the case due to monetary reasons or non-cooperation; the first non-compliance supposition that I had initially assumed has become a more far-reaching and more organized barrier. The accident has brought me to the point of thinking differently about the advocacy since the process of care coordination and structural support is not a choice but a compulsion. Second, the other areas of learning process that I have developed are my cultural competence and ethical responsibility. I have also been dealing with patients whose cultural or religious orientation was strong to the extent that they influenced the decision to provide care. The explanations of the methods of rumination and career development in my case provided the means of uplifting such situations by means of open communication and decision-making process. The ethics has also assisted me working as a nurturer and also as a healthcare system amplifier of the enormous and consolidated healthcare system.
Community Case Manager in Nursing.
After studying BSN440: Case Management, I have changed how I view the Case Manager position in the nursing community-based care. Needs assessment: Needs assessment elicits the role of a coordinator, advocate, educator, and liaison on a continuum of care, which is to be addressed by a Case Manager. The Case Manager will take part in ensuring a safe transition within the community context as the person will help in ensuring a safe transition among the hospitals, outpatient clinics, rehabilitation facility and home health services. The Case Managers provide a thorough evaluation of the needs to the patient (medical and psycho-social). They also collaborate within the interdisciplinary teams due to the development of individual care plans, tracking the progress, referral, and effective utilization of the resources also to be taken into account. The well-coordinated care is attributed to the sustainability and healthy patient outcomes in the health care environment.
Bid to Keep a Bedridden Patient Alive: Healthcare System Tour.
The patients referred to receive care coordination are lost in the healthcare system maze especially when it comes to the complex or chronic patients. One of the remarkable aspects of the input of the Case Manager in the coordination process of all services rendered by the providers and different levels of care is care continuity. However, circumstances such as heart failure may compel the patient to take medication, receive some diet education and follow-up, address the insurance issue, and get the help of the community. The Case Manager will make sure that they will be covered and exposed to the objectives of the patient. That being said, as Khatri et al (2023) posit, the fact that Care coordination in the primary health care system must be created is the primary concern, and that it will allow continuity and the capabilities of offering patients more effective results. This gap in care can be fulfilled by the case manager who identifies and eliminates the barriers whereby the barriers may be health literacy barrier, financial barrier or transportation barrier as the barriers enhance stability of the patient. It is systems approach; a leadership/ advocacy act, and a manifestation of consciousness of the world.
Comparison of a Disease Navigator and Case Manager.
The spheres of scope and focus are also different and the jobs in the Case Manager and Disease Navigator have certain overlaps. The various settings of care can be integrated to plan a multidisciplinary care of the various diagnoses with the aid of an A Case Manager. These were the discharge planning, interdisciplinary, and the long-term care management and systems navigation. Disease Navigator, in its turn, will have a specialization in a certain diagnosis (oncology or the treatment of diabetes). Disease Navigator is also geared up with the specialised education, emotional support, and remedy based on the disease route. Patient-centred care is based on the two functions. Nevertheless, the case Manager position is more general and systemic in contrast with the Disease Navigator which is a specialistic and diagnosis orientated one.
Conclusion
Another education that I received that led to my acceptance of the BSN program as a global citizen was the emphasis on the role of moral principles, cultural diversity, and structural issues in health care and subsequent impact on patient outcomes. The learned training under Case Management has contributed to the enhanced importance of continuity, coordination and advocacy of the community to the nursing profession. The nurses are supposed to be ethical human beings, culturally competent human beings, practitioners, and adopters of the value of fostering equitable and quality nursing services to all people as the health care systems continue to change.
