Nursing Practice Problem
High blood pressure is one of the most critical health concerns people are concerned about and a primary cause of heart diseases like heart failure and stroke. According to the view of the World Health Organization, hypertension is among the major causes of morbidity and mortality in the globe, particularly in the low- and middle-income nations. Most health care settings, such as urban populations, are faced with a lack of control regarding blood pressure due to a lack of proper patient education, medication compliance, and poor lifestyles. The proposed gap in effective management demonstrates that there is a need to implement improved nursing interventions in order to enable the patients to achieve improved health outcomes.
Population Health Database.
The World Health Organization reveals that hypertension is a major contributor to untimely death, and over a billion people in the world are affected by the disease. The rate of urbanization has caused the hypertension trend in Kenya and its surrounding conditions since it has led to lifestyle patterns such as poor dieting, failure to exercise, and high stress levels. In itself, it is a morbidity indicator since uncontrolled hypertension is a significant risk factor, which causes complications, such as stroke, kidney failure, and heart disease. Such statistics justify the necessity of applying hypertension as one of the priority nursing practice problems.
Population of Focus
The target population, which will be utilized in this project, will be represented by adult hypertension patients visiting outpatient or primary healthcare facilities. This group is particularly susceptible to complications in case blood pressure is not taken care of accordingly and requires frequent monitoring, training, and assistance from medical doctors.
Intervention
The proposed intervention is a properly structured nurse-guided lifestyle education. The program consists of patient education about nutritional change, including low salt, physical exercise, adherence to medicine, weight, and regular blood pressure monitoring. Some of the major ways of this intervention practiced by nurses include patient-centered education and follow-up assistance.
Comparison
This intervention is compared to the normal care that typically presupposes a regular clinical management and does not involve a planned and constant nurse-centered education program. Even the general advice may be assumed as standard one; however, without the systematic and continuous assistance, the behavior change will not be lasting.
Outcome
The resulting desired outcome is improved blood pressure control, which is indicated by a reduction in systolic and diastolic blood pressure. The alternative outcomes that may occur include increased patient education, increased compliance with treatment, and reduced risk of complications associated with hypertension.
Time Frame
It will take three months before some measurable growth can be achieved in the case of the increase of nurse-led lifestyle education programs.
PICOT Statement
The question is as follows: In hypertensive patients undergoing outpatient care (P), does the application of a structured nurse-led lifestyle education program (I) compared to standard care (C) in blood pressure regulation (O) in three months (T)?
Supporting Evidence
Evidence on the effectiveness of nurse-led interventions to improve the outcome of hypertension exists. The effectiveness of lifestyle change and education programs has been proven to be very effective in managing blood pressure as well as patient compliance with the therapy (Li et al., 2025).

The purpose of this assignment is to select a relevant nursing practice problem for your evidence-based practice project proposal. To identify a relevant problem, consider problems generally faced in nursing practice (coordination of health care, assessment, education, patient support, trauma prevention, recovery, health screenings, etc.). Use the “PICOT Draft” template to complete this assignment.
Use a national, state, or local population health care database to research indicators of disparity. Choose a mortality/morbidity indicator to identify a clinical problem or issue that you want to explore pertaining to a population of focus. Use this indicator to begin to formulate a PICOT (Population/Problem, Intervention, Comparison, Outcome, and Time to achieve the outcome) statement.
